Use the RSP Address tab on the UPS Canada Shipper Defaults page to enter the responsible party address for this UPS Canada account. A responsible party address is required on form B13A (the Canada Export Declaration form) and the responsible party is the person who signs the B13A.
The Canada Export Declaration (B13A) form is required for:
•Controlled, prohibited or regulated goods, regardless of their value, that are destined for consumption outside United States, Puerto Rico, or US Virgin Islands. Two copies of the form are required.
•Goods valued at $2,000 Canadian Dollars or more if the final destination of the goods is a country other than United States, Puerto Rico, or US Virgin Islands.
Note: If a shipment contains items grown, mined, or manufactured in different provinces, a separate form must be completed for each province.

Type the fax number of the responsible party. |

Type the phone number of the responsible party. |
Telex |
Type the telex number of the responsible party. |
Is Exporter |
If the responsible party is the exporter of the shipments, select this check box. |
Click [SAVE] in the lower-right corner of the screen to save any changes you made.
